About this template

The log is a portrait page with a row for each reading: date, time, upper (systolic) and lower (diastolic) numbers, pulse and a notes column. The weekly diary is a landscape sheet with morning and evening rows for each day. They are recording sheets only and do not explain or judge the numbers. Each comes as a blank and a worked example with made-up readings, in Word and PDF, free.

Questions

No. It only records the numbers. Your GP, nurse or pharmacist can explain them.

Which version should I use?

Use the log for an ongoing record and the weekly diary when you have been asked to record readings over a week.

Can I type into the sheet?

Yes. The Word version has a table you can type into. Print the PDF to write by hand.

Are the example readings typical or recommended?

No. They are made-up numbers to show how the sheet is filled in.
